The sequencing of the human genome and decreasing costs of sequencing technology have led to the notion of ‘personalized medicine’. This has been taken by some authors to indicate that personalized medicine will provide individualized treatments solely based on one’s DNA sequence. We argue this is overly optimistic and misconstrues the notion of personalization. Such interpretations fail to account for economic, policy and structural constraints on the delivery of healthcare. Furthermore, notions of individualization based on genomic data potentially take us (...) down the road of genetic reductionism obscuring the role of environmental factors in disease and ill health. We propose that one should see personalized medicine as a way of using personal genomic information to stratify individuals into subpopulations and suggest that personalized medicine be seen within a broader idea of personalized healthcare, reflecting healthcare that integrates personal genomic data into cultural, environmental and personal contexts. AimThe neurocognitive basis of Developmental Coordination Disorder remains an issue of continued debate. This combined systematic review and meta-analysis provides a synthesis of recent experimental studies on the motor control, cognitive, and neural underpinnings of DCD.MethodsThe review included all published work conducted since September 2016 and up to April 2021. One-hundred papers with a DCD-Control comparison were included, with 1,374 effect sizes entered into a multi-level meta-analysis.ResultsThe most profound deficits were shown in: voluntary gaze control during movement; cognitive-motor integration; practice-/context-dependent (...) motor learning; internal modeling; more variable movement kinematics/kinetics; larger safety margins when locomoting, and atypical neural structure and function across sensori-motor and prefrontal regions.InterpretationTaken together, these results on DCD suggest fundamental deficits in visual-motor mapping and cognitive-motor integration, and abnormal maturation of motor networks, but also areas of pragmatic compensation for motor control deficits. This paper argues that professionals who make boundary-related decisions should be guided by relationship-based practice. In our roles as service users and professionals, drawing from our lived experiences of professional relationships, we argue we need to move away from distance-based practice. This includes understanding the boundary stories and narratives that exist for all of us – including the people we support, other professionals, as well as the organisations and systems within which we work. When we are dealing with professional boundary (...) issues, we should centre relationship-building skills that are central to many other aspects of our work. Skills that foster relationships at all levels – between professionals, service users, and services – need to be revalued. Our final recommendation is to create, develop, and foster safer spaces within and outside of organisations, as well as inter-professionally, for the discussion and exploration of boundary-related issues and practice. We are interested in hearing from those with experiences of being marginalised by boundaries so that they can inform a reshaping of our collective ideas around boundary related practices. To foster relationship-based practices in organisations, we have outlined several recommendations here; however, we recognize that these do not go far enough, and that collective action is needed to inform systemic change. Background: Expanded newborn screening generates incidental results, notably carrier results. Yet newborn screening programmes typically restrict parental choice regarding receipt of this non-health serving genetic information. Healthcare providers play a key role in educating families or caring for screened infants and have strong beliefs about the management of incidental results. Methods: To inform policy on disclosure of infant sickle cell disorder (SCD) carrier results, a mixed-methods study of healthcare providers was conducted in Ontario, Canada, to understand attitudes regarding result management (...) using a cross-sectional survey (N = 1615) and semistructured interviews (N = 42). Results: Agreement to reasons favouring disclosure of SCD carrier results was high (65.1%–92.7%) and to reasons opposing disclosure was low (4.1%–18.1%). Genetics professionals expressed less support for arguments favouring disclosure (35.3%–78.8%), and more agreement with arguments opposing disclosure (15.7%–51.9%). A slim majority of genetics professionals (51.9%) agreed that a reason to avoid disclosure was the importance of allowing the child to decide to receive results. Qualitatively, there was a perceived “duty” to disclose, that if the clinician possessed the information, the clinician could not withhold it. Discussion: While a majority of respondents perceived a duty to disclose the incidental results of newborn screening, the policy implications of these attitudes are not obvious. In particular, policy must balance descriptive ethics (ie, what providers believe) and normative ethics (ie, what duty-based principles oblige), address dissenting opinion and consider the relevance of moral principles grounded in clinical obligations for public health initiatives. This article gives new information about the social, cultural, and political beliefs of Thomas Wilson, the first Secretary and Treasurer of the Yorkshire Geological and Polytechnic Society founded in 1837. Wilson's private correspondence was not available to J. W. Davis, who wrote the standard history of the Society to celebrate its jubilee. Davis's history underestimates the part played by Wilson and overestimates the importance of Thomas William Embleton, whose private papers were placed at Davis's disposal. It is (...) shown that Wilson, and not Embleton, was the driving force in forming and consolidating the Society, especially with respect to its early difficulties in defining even its title and aims. In the opening sections of his Brahma-sutra-bhasya , Ramanuja makes a very forceful assault on Sankara's Advaita theory. This assault anticipates in a striking way modern western attacks on metaphysical religious positions, attacks which stem from Hume and are associated today with names like A. J. Ayer and Antony Flew. In this paper I wish to argue that certain aspects of Sankara's position, as enunciated in his Brahma-sutra-bhasya , suggest that Ramanunja's assault, and therefore by implication a modern western attack (...) on Sankara also, depends for its success on a misinterpretation of Sankara's views. I wish then to suggest a possible alternative interpretation of Sankara's Advaita in terms viable for today, drawing, though indirectly, on such writers as R. B. Braithwaite and R. M. Hare. An incidental implication of this paper is that these two philosophers of religion would meet less opposition from the religious if they were Hindu rather than Christian! A more important implication is that light could be thrown on the present religious controversy in the west by a study of Indian philosophy of religion, if only we were more disposed to treat it as a living tradition which might teach us something, rather than as an antique. We discuss how modified dual-task approaches may be used to verify the degree to which cognitive tasks are capacity demanding. We also delineate some of the complexities associated with the use of the “double easy-to-hard” paradigm for testing claim of Halford, Wilson & Phillips that hierarchical reasoning imposes processing demands equivalent to those of transitive reasoning.

Introduction Future HIV vaccine efficacy trials with adolescents will need to ensure that participants comprehend study concepts in order to confer true informed assent. A Hepatitis B vaccine trial with adolescents offers valuable opportunity to test youth understanding of vaccine trial requirements in general. Methods Youth reviewed a simplified assent form with study investigators and then completed a comprehension questionnaire. Once enrolled, all youth were tested for HIV and confirmed to be HIV-negative. Results 123 youth completed the questionnaire (mean age=15 (...) years; 63% male; 70% Hispanic). Overall, only 69 (56%) youth answered all six questions correctly. Conclusions Youth enrolled in a Hepatitis B vaccine trial demonstrated variable comprehension of the study design and various methodological concepts, such as treatment group masking. (shrink)

The evidence of ancient books points to the surprising conclusion that in texts of drama or prose dialogue changes of speaker were not usually marked by the name of the new speaker. Instead the ancient reader had a colon, sometimes combined with a paragraphus or stroke in the margin, to guide him. The inconvenience of this practice and the muddle it caused need no emphasis. Unlike most other sciences, psychology has no true core theory to guide a coherent research programme. It does have James J Gibson’s ecological approach to visual perception, however, which we suggest should serve as an example of the benefits a good theory brings to psychological research. Here we focus on an example of how the ecological approach has served as a guide to discovery, shaping and constraining a recent hypothesis about how humans perform coordinated rhythmic movements (Bingham 2004a, b). Early (...) experiments on this task were framed in a dynamic pattern approach. This phenomenological, behavioural framework (e.g. Jeka & Kelso 1989) classifies the behaviour of complex action systems in terms of the key order parameters, and describes the dynamical stability of the system as it responds to perturbations. Dynamical systems, however, while a valuable toolkit, is not a theory of behaviour, and this style of research is unable to successfully predict data it is not explicitly designed to fit. More recent work by Bingham & colleagues has used dynamical systems to formalise hypotheses derived from Gibson’s ecological approach to perception and action, with a particular emphasis on perceptual information. The resulting model (Bingham 2001, 2004a, b; Snapp-Childs et al. 2011) has had great success with both the phenomena it was designed to explain as well as a wide range of empirical results from a version of the task it is not specifically designed to explain (specifically, learning a novel coordination). This model and the research programme that produced it stand as an example of the value of theory driven research, and we use it to illustrate the contemporary importance the ecological approach has for psychology. How should we? In the early twentieth century, these most foundational questions relating to music education were addressed by the highly regarded, though less well known, educator and researcher, Otto Rudolph Ortmann. In 1922, he published an article in which he outlined a theory of musical experience, developing aspects (...) of the physics of sound, qualitative theory, and Gestalt psychology, prefiguring important ideas in music philosophy. Ortmann highly valued his theory, utilized it in other research throughout his career, and influenced others such as Carl Seashore. Few, however, seem to be aware of his work or its implications for music education.1The seminal 1922 article, "The Sensorial Basis of Music Appreciation," was written while Ortmann was teaching at the Peabody Conservatory of Music and Johns Hopkins University in Baltimore, Maryland.2 While this article has received little attention, much of his other research has been widely and well received.3 "Papa Ortmann," well beloved of family and students, had a career Gerig characterized as "a wedding between Materia Medica and Frau Musica."4 A prolific [End Page 160] researcher, superb musician, and excellent teacher, he published articles on music theory (one of which was reprinted and extended in 1983), musicology, music reading, ear training, synesthesia, and jazz.5 His research on piano performance, particularly his two books,6 is regarded as definitive; it has been discussed in a thesis and four dissertations; some of it has been replicated. In Famous Pianists and Their Technique, Gerig devoted a chapter to Ortmann.7 Ortmann's theory of listening types, in effect a theory of music cognition,8 was accepted by Robert Neidlinger, Doreen Rao, and Bennett Reimer.9 In the 1992 Handbook of Research on Music Teaching and Learning, F. R. Wilson and F. L. Roehmann praised the significance of Ortmann's body of research as undiminished in importance in music education.10It may be that Ortmann's "Sensorial Basis"theory received little attention at the time because too few were interested in such theories or in the synthesizing of science and psychology or the touching on philosophy that he employed. Although he influenced Seashore, the philosophical issues he began to explore would be developed more strongly only decades later, by philosophers such as Monroe Beardsley, Peter Kivy, and Jerrold Levinson. It seems that Ortmann's theory not only prefigures some ideas in recent music philosophy, but supports them as well. If it does, then our increased understanding of music can provide a firmer basis for developing better music education-one of Ortmann's aims. In this article I will describe his theory, sketch how it aligns with subsequent research, and consider its value today. Ortmann attempted a comprehensive explanation of music's nature; as far as his explanation is accurate, that far it can help us understand music and in turn music education.Three "Attributes"At the turn of the 1900s, it was popular to refer to "attributes" of sound. Ortmann posited that all sensation, including sound, has three types of attributes. The primary attribute he termed extensity, by which he meant the breadth of the sensation that is perceived. Extensity has three forms, transtensity (frequency), intensity (strength), and protensity (duration). These can be perceived individually but also as sound's secondary attributes, chief of which is quality. Tertiary attributes of sound are made through association, either by (a) contiguity, linking a sound's quality with things experienced near in time to the sound, or by (b) similarity, associating the quality with sensations of like extensity (for example, the expressive brightness of a sound and a light). Ortmann believed that tertiary attributes form "the threshold of musical imagination, and imagination in turn, of musical enjoyment."11 Accordingly, he concluded that music could express many things, and that this is dependent on listeners, their individual histories, and their entire field of sensations. Individuals are a prominent part of the biological world. Although biologists and philosophers of biology draw freely on the concept of an individual in articulating both widely accepted and more controversial claims, there has been little explicit work devoted to the biological notion of an individual itself. How should we think about biological individuals? What are the roles that biological individuals play in processes such as natural selection (are genes and groups also units of selection?), speciation (are species individuals?), and (...) organismic development (do genomes code for organisms)? Much of our discussion here will focus on organisms as a central kind of biological individual, and that discussion will raise broader questions about the nature of the biological world, for example, about its complexity, its organization, and its relation to human thought. Leibnizian metaphysics underpins the universally held view that spacetime must be inextendible – that it must be “as large as it can be” in a sense. But here we demonstrate a surprising fact within the context of general relativity: the property of inextendibility turns out to be unstable when attention is restricted to certain collections of “physically reasonable”spacetimes.

Influential cases have been provided that seem to suggest that one can fail to have knowledge because of the social environment. If not a distinct kind of social defeater, is there a uniquely social phenomenon that defeats knowledge? My aim in this paper is to explore these questions. I shall argue that despite initial appearances to the contrary, we have no reason to accept a special class of social defeater, nor any essentially social defeat phenomenon.
